How Is Abdominoplasty Performed?

Abdominoplasty, or tummy tuck, is a major cosmetic plastic surgery procedure that is typically performed under general anesthesia, where you will sleep through the procedure, or your surgeon may also use a local anesthetic combined with a sedative. For local anesthesia, some sensation and discomfort may occur. A full tummy tuck procedure can require from 2 to 5 hours of surgical time.

The Abdominoplasty Incision and Procedure

The most typical surgical technique for abdominoplasty begins with a long incision across the abdomen, from hip bone to hip bone. The shape of the incision resembles a V, only with a flat bottom. The flat part of the V occurs at the bottom point of the abdomen immediately above the pubic region. The sides of the V form angles along the pelvis straight up to the top of the hip bone. Another incision is made around the navel to free it from the abdominal tissue.

The skin is separated from the abdominal wall, up to the ribs, exposing the vertical abdominal muscles after the incisions are made. The abdominal muscles are then surgically stitched into a new position, thereby tightening the muscles and reducing the waistline. The excess skin is stretched and removed after the muscles have been repositioned and tightened.

The remaining skin is then replaced over the abdominal area and stitched into place. The cavity created by the incision around the belly button is generally removed when the excess skin is cut away, and a new hole is formed to properly position the navel.

Partial Abdominoplasty

The partial abdominoplasty is a procedure in which a smaller incision is made and no tissue is removed around the navel. The belly button is normally left in place or can be repositioned as with other techniques. This procedure is performed when there is a very localized fullness in the lower abdomen, so that less extensive muscle repair and skin removal is needed. The partial abdominoplasty requires 1 to 2 hours of surgery.

After Abdominoplasty

Abdominoplasty is a major surgical procedure and requires a considerable amount recovery time compared to other cosmetic plastic surgical procedures. Many patients spend 1 to 2 days in the hospital, but this may not be necessary for you depending on the extent of the procedure. For the first several days, the abdomen will be swollen and sore, and the patient will have some discomfort, which can be properly managed with medications.

Many people require only one to three weeks before returning to work and a normal schedule. Only after sufficient healing time has passed is exercise recommended after surgery, but the exercise should be performed comfortably.

The abdominal scarring will appear worse during the first three to six months, and may take nine months before they flatten out and lighten in color. These scars never completely disappear; however, due to their location, they are usually well hidden by clothing and even bathing suits.

Abdominoplasty Alternatives

Abdominoplasty is performed to change the body shape and contour, not to reduce weight. If fat is localized in the abdomen, but no excessive skin exists, then liposuction is an alternative to abdominoplasty. Normally, liposuction and abdominoplasty are not done at the same time because suction of the skin during liposuction may damage the skin that is being stretched and repositioned.

Obese patients may undergo a modification of the abdominoplasty procedure called panniculectomy.
